Sanctuary Personnel, an innovative and committed recruitment agency is currently looking for a dedicated and experienced Social Work Team Manager to work based in Preston, within the Adult Community Team.
The pay rate for this locum Social Work Team Manager role is £20.01 per hour (Assignment rate) and is a 3-month contract.
Main Duties:
Contribute to service delivery that is person centred and outcome focussed with an emphasis on encouraging independence and enabling recovery.
Work collaboratively with other professionals and agencies to address complex needs and achieve best outcomes for individuals and their carers/families.
Ensure that the assessed social care needs of individuals are met within the requirements of legislation and available resources, focussing on the strengths of the individual and the promotion of independence.
Co-ordinate a comprehensive holistic assessment of need and risk.
Establish and review an initial support plan that is agreed with the service user, carer(s) and service providers.
Undertake social care assessments, support planning and reviews in a timely manner. To ensure the provision of high-quality services to individuals through creative support planning which identifies measurable outcomes for service users and carers.
Utilise Liquid Logic (LAS) and other computer systems, for inputting assessment data; allocation and management of a caseload; recording of all contacts in relation to an allocated case; support plan and request the commissioning of appropriate services; refer to internal and external organisations; demonstrate availability for undertaking assessment/review visits.
Requirements of the Social Work Team Manager:
Degree or equivalent in social work.
Current Social Work England registration.
Recent post qualifying experience within frontline adult social work, including experience of working within a social care setting, with a range of service users, and assessing the needs of individuals and their families.
Full driving licence and access to personal vehicle at their disposal.
