What you will do:
- Conduct detailed, person-centred assessments of adults needing care, ensuring their needs are accurately identified.
- Develop, implement, and monitor personalised care plans that enhance independence and well-being.
- Collaborate with health professionals, support organisations, and community resources to coordinate effective interventions.
- Provide safeguarding measures to protect vulnerable adults, closely following both local and national safeguarding policies.
- Maintain accurate and up-to-date case records, producing high-quality reports as needed.
- Offer valuable advice and guidance to individuals and their families about available services and support networks.
- Ensure all work is compliant with relevant legislation, including the Care Act 2014 and Adult Support and Protection (Scotland) Act 2007.
Requirements:
- Hold a Degree in Social Work and registration with Social Work England (SWE) or the Scottish Social Services Council (SSSC).
- Demonstrate experience in adult social work, with a focus on assessments, care planning, and safeguarding.
- Possess an excellent understanding of the legislation, policies, and procedures concerning adult social care in Scotland.
- Showcase outstanding communication and interpersonal skills to effectively engage with service users and professionals.
- Exhibit the ability to work independently, manage workloads efficiently, and make informed decisions under pressure.
