What you will do:
- Provide high-quality, research-based casework service to adults living in the Bailiwick of Guernsey.
- Conduct assessments using standardised formats and create personalised care plans for individuals.
- Work as part of an integrated adult community care team offering consultation, support, and specialist advice.
- Coordinate services for adults wishing to remain in their own homes or transition into care.
- Develop relationships with service users and accurately assess complex psychosocial needs.
- Provide complex psychosocial support for those with terminal illnesses and their families.
- Offer information, support, and signposting related to housing, benefits, finances, and care options.
- Ensure compliance with safeguarding policies and address the needs of vulnerable adults.
- Participate in multi-professional meetings and contribute to holistic assessments.
