What you will do:
- Conduct comprehensive Care Act Assessments in preparation for DST meetings
- Perform Mental Capacity Assessments to support individual care plans
- Manage commissioning work for individuals transitioning out of CHC eligibility
- Collaborate with healthcare professionals to deliver person-centred care
- Maintain accurate records and documentation using the Adults Liquid Logic system
- Provide expert advice and support within the Adults CHC team
*The Social Work position requires recent experience in undertaking CHC-related work. As part of this role, you will be responsible for completing Care Act Assessments prior to Decision Support Tool (DST) meetings, conducting Mental Capacity Assessments, and handling related commissioning work for individuals who are no longer eligible for CHC. A thorough understanding of the NHS Continuing Healthcare Framework is required, as is practical experience with the Adults Liquid Logic (LAS) IT system.
