Sanctuary Personnel, an innovative and committed recruitment agency is currently looking for a dedicated and experienced Social Work Trusted Assessor to work based in Bexley.

The pay rate for this locum Social Work Trusted Assessor role is £15.36 per hour (Assignment rate) and is a minimum 2-month contract.

Working as a “Trusted Partner” based in Mind in Bexley you will enable people to find the right support services that meet their needs and fulfil their personal goals – supporting them to maintain their independence for as long as possible and helping them to plan ahead. This project aims to ensure that where people may need additional support this can be provided in an effective and timely way to help maintain their independence and quality of life. You will achieve this by developing plans that are person-centred and holistic and which support the well-being of people with varying needs and levels of ability at different stages in their lives.

Working Hours:

  • 21 hours per week.
  • The post holder may be required to work flexibly to meet the needs of the service.

Main Duties:

  • Provide people and their families with information and guidance that enables them to explore innovative and creative ways to meet their needs, find solutions and achieve their personal outcomes.
  • Assess a person’s support needs by developing an understanding of their circumstances, personal strengths, and aspirations along with any family or community networks they have.
  • Work with the person to help them develop their own person-centred support plan which takes a preventative approach and incorporates their own choices and a positive approach to taking risks.
  • Help people to navigate the Health and Care system and maintain a good understanding of the provisions of the Care Act 2014 and other relevant legislation (such as the Mental Capacity Act 2005), so that a person is given clear information in relation to the assessment being carried out.
  • Explain to a person their own responsibilities and those of the Council’s Adult Social Care (ASC) department in relation to implementing a Care Act support plan. This will include an explanation that the person’s assessment information and support plan will be recorded on the Council’s record system, how their support plan is costed, the process the Council follows to work out any financial contribution the person will need to make and describing the process and enabling the person to understand the Council’s arrangements for self-directed support, personal budgets and Direct Payments.
  • Work alongside people to review their care or support plan if and as their needs change.
  • Ensure that information about the person’s assessed needs and their support plan is recorded accurately and in a timely way on the relevant management information systems.
  • Support people to access other services when appropriate; this may include making a referral to other Health or statutory Social Care if particular needs are identified as part of the assessment or support planning process or when a person’s needs are more complex or become unsettled.

Requirements of the Social Work Trusted Assessor:

  • Degree or equivalent in social work.
  • Current Social Work England registration.
  • Experience of working with older people, adults with disabilities or mental health needs and with family carers, experience in support planning through working with individuals one-to-one, and experience of working with people from different backgrounds in an engaging way which is responsive to their own lived experience.
  • Experience of working with other health and care colleagues, experience of using a case/client management system for recording purposes, and experience of working within social care legislation (Care Act 2014 and Mental Capacity Act 2005) and having a willingness to develop this knowledge.
  • Knowledge and understanding of the issues that affect older people, people with disabilities, and people with mental health needs and the impact these conditions can have on their lives and on the lives of those around them.
  • A good understanding of the Care Act 2014 and how to use it in practice.
  • Excellent communication, observational and interpersonal skills with the ability to adapt personal style to the needs of different audiences.
  • Ability to use computer technology effectively with confidence in MS Outlook, Word and Excel.
  • Good written and numeracy skills with the confidence to manage data.
  • Willingness to work in a variety of settings across London Borough of Bexley
