What you will do:
- Assess the needs of adults requiring social services and develop comprehensive support plans to meet those needs
- Conduct face-to-face visits and build strong relationships with service users and their families ensuring they receive suitable, personalised care
- Collaborate with other professionals such as healthcare providers and community staff to ensure a holistic approach to social work and adult care
- Maintain accurate records and prepare reports to support decision making in social care services
- Provide guidance and support to adults in need of assistance facilitating access to appropriate resources and services
- Uphold safeguarding practices and remain informed of any changes in policy or best practices in adult social work
