Sanctuary Social Care is currently looking for an experienced and dedicated Social Care Assessor to work full-time in Wandsworth, within the Mental Health Social Care Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Care Assessor job is £27.57 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Duties of the Social Care Assessor:
- Undertaking Care Act assessments
- Undertaking Carers assessments
- Completing collaborative support plans, applying for funding and arranging for services
- Liaising with acute inpatient mental health wards to provide advice and signposting around referrals for adult social care
- Completing screening of needs where health colleagues are unsure whether to complete a referral for a Care Act assessment
- To respond in a timely way to facilitate discharge from hospital and avoid delays to this
- To take part in the team duty rota
Requirements of the Social Care Assessor:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
