Sanctuary Personnel, an award-winning and renowned Recruitment Agency is looking for an experienced and dedicated Social Care Assessor to work based in North Yorkshire.
The pay rate for this Social Care Assessor job is £20.68 per hour (assignment rate) and is an initial 2-month contract.
Main duties:
- Working closely with colleagues in the CCG to determine eligibility for Continuing Healthcare. These assessors can be Nurses, Occupational Therapists or Social workers
- Predominately be a member of the multidisciplinary team to support the completion of the decision support tool and to agree an eligibility decision
- Complete care act assessments and to be able to determine what needs may be beyond that which social care can provide
- Complete and/or update care act assessments
Requirements of the Social Care Assessor:
- Degree or equivalent within Social Work
- Experience and knowledge of Continuing Healthcare as well as the Care Act is essential
- Welcoming applications from nursing, occupational therapy and social work professions
