Key Responsibilities:
- Conduct thorough assessments to address mental health and social care needs, ensuring timely hospital discharges.
- Apply your expertise in statutory duties to manage a complex caseload efficiently.
- Collaborate with carers and professionals to achieve the best outcomes for individuals.
- Perform social care needs assessments, Human Rights assessments, and mental capacity assessments.
- Act as a Safeguarding Enquiry Officer, working closely with the Safeguarding Adults Manager to ensure safe outcomes.
- Advocate for equality and the effective application of health and social care policies.
Candidate Requirements:
- Excellent communication and assessment skills.
- A passion for supporting carers and individuals in the mental health sector.
- Strong knowledge of adult social care and mental health legal frameworks.
- An Approved Mental Health Professional (AMHP) Qualification is highly desirable.
