Sanctuary Social Care has permanence vacancy for a Senior Practitioner to work full-time based in Southend-on-Sea, within the Mental Health Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The salary for this permanent Senior Practitioner role is £37,849 - £43,662 per annum.
Duties of the Senior Practitioner:
- Undertake Care Act assessments and will support and review adults with a Learning Disability via Self Directed Support with a view to supporting them to achieve greater levels of independence and to achieve agreed outcomes. You will have significant support from the Team Manager, 2 Practice Leaders and social work colleagues, many of whom have significant experience of working with adults with a Learning Disability.
- To use a strengths-based approach to social work practice and will work closely with all partners across Health and Social Care.
- To ensure that there is effective coordination of interdependence including the management of and resolution of risks and other issues that may arise.
Requirements of the Senior Practitioner:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within adults' services
- Must have excellent awareness of self and others and be able to work effectively with a range of staff groups and partners across the system.
- Previous experience in Adults Knowledge of the Care Act, Mental Capacity Act, self-directed support, Liquidlogic, NHS Continuing Healthcare and undertaking safeguarding investigations are essential.
