Sanctuary Social Care currently has a locum vacancy based in Kensington and Chelsea for a Senior Practitioner to work full time within a Complex Care Team. The ideal candidate will possess highly developed specialist knowledge of Social Work Practice in Adult Services and be registered with the HCPC.
The pay rate for this locum practitioner role is £34.50 per hour (Assignment Rate) and is a 3 month contract.
Duties:
Assessing clients under the Care Act.
Participating in joint assessments with health.
Participating in Decision support tool meetings.
Challenging decisions where required.
Supporting clients to return home using strengh based assessments and support planning.
Taking complex cases to panel for social care funding.
Reviewing clients under temporary health funding.
Hold and manage a complex case load.
Requirements of the Senior Practitioner:
Degree or equivalent in social work
Current Social Work England registration
Significant senior level frontline experience within Adult's services
Must have good knowledge of Care Act and NHS CHC policy and procedures.
We require someone with excellent assessment skills and ability to create robust care plans.
Candidate must have expirence of working in MDT settings.
Candidate must feel confident in challenging CHC decision where required.
