Sanctuary Personnel, a committed and competitive recruitment agency is currently looking for an experienced and dedicated Social Work Senior Practitioner to work full-time based in Tower Hamlets, within the Adults Initial Assessment Team.

The pay rate for this locum Senior Practitioner role is £34.40 per hour (Assignment rate) and is a 3-month contract.

Duties:

  • Support people to live independently, safely and healthily in the community. IA is the first point of contact/access for Tower Hamlets Adult Services
  • Processing all new requests for assessment, provision of equipment to facilitate safe hospital discharges/avoid bed blocking, initiate the failed visit procedure for cases held in IAT and provide general information, advice and signposting around social care issues including those cases open to localities/key workers
  • Be the first point of contact for concerns, queries and enquiries related to Adult Social Care in the London Borough of Tower Hamlets
  • Process and triage all safeguarding concerns including risk assessments, protection planning & onward referral if sec 42 Enquiry is required

Requirements of the Senior Practitioner:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant senior level frontline experience within adults’ services
