Sanctuary Personnel, an award-winning and renowned Recruitment Agency is looking for a Band 6 Senior Mental Health Practitioner (CPN) to work based in Ince-in-Makerfield.

The pay rate for this full-time role is £24.32 - £25.23 per hour (Assignment rate) and is an initial 4-month ongoing contract.

Duties:

  • Manage the Core waiting lists in terms of allocation, escalation and re allocation
  • Provide direct contact with those waiting, including risk reviews, mental state reviews and referral to group psychological interventions for the people referred into the service
  • Undertake risk assessments and care plans accordingly
  • Work in ways that are sensitive to the needs of people from a wide range of racial, cultural and religious backgrounds
  • Be responsible for the screening of cases referred into the service
  • Participate directly in the teams duty rota’s
  • Exercise responsibility and autonomy, within supervisory, managerial and accountability structures, for the assessment, treatment and discharge of clients
  • Deliver care co-ordination, ensuring effective assessment, care planning and risk management processes are maintained
  • Involve the views of patients, referring agents and other professionals involved in the network of care including organising multi-agency meetings
  • Produce a clinical formulation and intervention plan founded on evidence based practice in co-operation with the patient
  • Hold responsibility for standards of clinical practice within area of specialist knowledge
  • Work jointly and collaboratively with other team members in order to enhance and develop the provision of services to patients
  • Have an effective knowledge base of other modalities so as to engage and work effectively with colleagues in other disciplines
  • Provide consultation, advice and guidance to junior colleague
  • Promote effective multi-agency work
  • Contribute theoretical knowledge to the process of managing risk within the multi-agency network
  • Undertake risk management for all patients including the protective and risk factors present in the network of significant relationships
  • Identify and respond appropriately to protection issues, as described with the guidelines of Local Safeguarding  Board and Greater Manchester Mental Health NHS Foundation Trust safeguarding policies
  • Maintain clear and comprehensive electronic clinical records, to ensure information is accurate and complies with Data Protection Act and professional body and Trust standards
  • Engage with using routine outcome measures as clinically appropriate including their use in clinical practice and inputting data into clinical records
  • Work collaboratively with patients and contribute to the trust’s service user participation agenda
  • Attend the core allocation meeting and provide a summary of the waiting lists including priority order for those referred into our service

Requirements of the Senior Mental Health Practitioner:

  • Valid NMC registration under Mental Health
  • Prior experience working in this environment
  • A degree or equivalent within Mental Health Nursing (RMN trained)
