Sanctuary Health has a permanent job vacancy for an experienced and dedicated Band 7 Senior Community Practitioner to work full-time based in Guernsey. The ideal candidate will possess highly developed specialist knowledge of nursing and be registered with the NMC.
The salary for this permanent Senior Community Practitioner role is £41,956 - £55,315 per annum.
Role Purpose:
- In conjunction with other members of the multi-disciplinary team they will be responsible for the delivery of mental health care to service users within the community, following a model of care which promotes close team work, and liaison/partnership with other mental health services as required.
Main Duties/Responsibilities:
- Be responsible for the assessment and care management of service users in a community setting.
- Manage a full caseload and co-ordinate care for their service users.
- Carry out comprehensive risk assessment and the formulation of management plans for service users in a community setting.
- Liaise with other health care professionals including, Psychiatrists, social workers, GP's, Probation, Prison staff, Children's services, In-patient MH services, Emergency Department, Third Sector, Healthy Minds amongst others as required.
Requirements of the Senior Community Practitioner:
- Registered Practitioner with NMC or HCPC registration.
- Basic and advanced Competence / qualification in Endoscopy Care and Decontamination.
