Role Purpose:
- Deliver high-quality social work practices within the Integrated Neighbourhood Team.
- Assess, plan, and implement person-centred care to promote autonomy and safeguard vulnerable adults.
Key Responsibilities:
Assessment and Care Planning:
- Conduct comprehensive assessments in line with the Care Act 2014.
- Develop and maintain personalised care plans focusing on independence and well-being.
- Collaborate with health professionals and stakeholders for integrated, multi-disciplinary support.
Case Management:
- Manage a complex caseload, delivering timely and effective interventions.
- Ensure accurate and compliant case records that meet statutory and legal standards.
- Regularly monitor and review care plans, adjusting strategies as required to achieve optimal outcomes.
Safeguarding:
- Respond promptly to safeguarding concerns, ensuring vulnerable adults are protected.
- Participate in safeguarding enquiries and engage in multi-agency safeguarding meetings.
Collaboration and Partnership:
- Work closely with healthcare, housing, voluntary organisations, and community groups to deliver holistic care.
- Engage service users and their families, incorporating their input into care plans and decision-making.
Professional Development:
- Stay up-to-date with legislation, policies, and best practices in adult social care.
- Engage in regular supervision and participate in professional development opportunities.
Requirements:
Essential:
- Degree in Social Work or an equivalent qualification.
- Registered with Social Work England.
- Proven experience in statutory adult social care settings.
- In-depth knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
- Strong assessment and communication skills with the ability to manage complex caseloads effectively.
Desirable:
- Experience working in integrated health and social care environments.
- Practice Educator qualification for the Senior Social Worker role.
