We are seeking a Senior Adult Social Worker to join our Integrated Neighbourhood Team within Adult Social Care in a vibrant London borough. Role Purpose:

  • Deliver high-quality social work practices within the Integrated Neighbourhood Team.
  • Assess, plan, and implement person-centred care to promote autonomy and safeguard vulnerable adults.

Key Responsibilities:

Assessment and Care Planning:

  • Conduct comprehensive assessments in line with the Care Act 2014.
  • Develop and maintain personalised care plans focusing on independence and well-being.
  • Collaborate with health professionals and stakeholders for integrated, multi-disciplinary support.

Case Management:

  • Manage a complex caseload, delivering timely and effective interventions.
  • Ensure accurate and compliant case records that meet statutory and legal standards.
  • Regularly monitor and review care plans, adjusting strategies as required to achieve optimal outcomes.

Safeguarding:

  • Respond promptly to safeguarding concerns, ensuring vulnerable adults are protected.
  • Participate in safeguarding enquiries and engage in multi-agency safeguarding meetings.

Collaboration and Partnership:

  • Work closely with healthcare, housing, voluntary organisations, and community groups to deliver holistic care.
  • Engage service users and their families, incorporating their input into care plans and decision-making.

Professional Development:

  • Stay up-to-date with legislation, policies, and best practices in adult social care.
  • Engage in regular supervision and participate in professional development opportunities.

Requirements:

Essential:

  • Degree in Social Work or an equivalent qualification.
  • Registered with Social Work England.
  • Proven experience in statutory adult social care settings.
  • In-depth knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
  • Strong assessment and communication skills with the ability to manage complex caseloads effectively.

Desirable:

  • Experience working in integrated health and social care environments.
  • Practice Educator qualification for the Senior Social Worker role.
