Sanctuary Health is currently looking for an experienced and dedicated Rehabilitation Support Worker to work full-time or part-time in Bristol.
The pay rate for this locum Support Worker job is dependent on experience (Assignment rate) and is a 3-month contract. To work variable days and shifts over 7 days between the hours of 08:00am -20:00pm in order to meet the needs of the service
Job Summary:
- The post holder works as part of the Home First team which is a citywide team supporting service users in their own homes with personal care needs and activities of daily living over a short term period of assessment and rehabilitation following discharge from hospital.
Duties:
- Undertake a range of delegated clinically led activities, having received training and been assessed as competent.
- To carry out the agreed rehabilitation plan in accordance with instructions.
- Following prescribed aspects of care, supervising and assisting patients where necessary with personal and domestic living activities.
- Work within your level of competence, only performing those tasks you have been trained to do and feel competent to carry out.
- Escalate any changes in patient presentation or concerns you have to a more senior member of your team.
Requirements of the Support Worker:
- Recent experience within a similar role.
- Experience of direct patient care in a health care and/or social care setting.
- Experience of team working.
- Experience of working without direct supervision.
- Experience of working across organisational boundaries within health or social care.
- An understanding of the role of intermediate care, and the wider NHS.
- An understanding of the law relating to data protection/confidentiality.
- An understanding of the implications of cultural difference for service delivery.
- Full driving licence and access to car in work time in order to travel independently across Bristol.
