You will be working on an ongoing basis, providing specialized care to individuals living with dementia and enduring mental illnesses. What You Will Do:
- Assess, develop, implement, and evaluate care plans using evidence-based standards.
- Operate autonomously and know when to escalate care concerns.
- Provide guidance and direction to other staff in care delivery.
- Support patients in daily living activities as outlined in care plans.
- Act as a health promoter and educator for patients and their networks.
- Report incidents and participate in quality and risk management processes.
- Maintain therapeutic relationships and respect communication needs.
- Collaborate effectively with multi-disciplinary teams.
- Serve as a professional role model and take charge when needed.
- Employ strong planning and organisational skills to manage tasks and resources.
