What you will do:
- Provide direct support to individuals with substance misuse issues, helping them to make positive changes in their lives.
- Work collaboratively with a multi-disciplinary team to develop and implement personalised recovery plans.
- Conduct assessments and monitor clients’ progress, adjusting support plans as necessary.
- Deliver one-on-one and group sessions to educate and empower clients about substance misuse and recovery strategies.
- Coordinate with local services and agencies to provide additional support tailored to each client’s needs.
- Maintain accurate and up-to-date client records in accordance with organisational standards and protocols.
