Sanctuary Personnel, an award-winning and renowned Recruitment Agency is looking for an experienced and dedicated Band 6 Reablement Physiotherapist to work full-time based in Dartford.
The pay rate for this locum Reablement Physiotherapist job is £28.50 per hour (Assignment rate) and is a 2-month contract.
Main Duties:
- Work as part of a multidisciplinary team, including health and social services colleagues to meet the needs of clients referred to the Reablement Team.
- Conduct Physiotherapy assessment for clients receiving input from the Reablement Team.
- Provide Physiotherapy treatment and rehabilitation plan for clients with the Reablement Team ensuring that support workers are able to follow the rehabilitation plan safely and efficiently with clients.
- Work within the multidisciplinary team as an autonomous practitioner, ensuring the highest possible standards of practice.
- Monitor, review, and continually reassess client care plans and objectives against outcomes measured in order to facilitate modification of packages of care/targets as appropriate to enable achievement of their goals.
Requirements of the Reablement Physiotherapist:
- Degree or equivalent within Physiotherapy.
- HCPC registered as a Physiotherapist.
- Recent post qualifying experience working as a Physiotherapist.
