Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Southwark, within the Adult's rehab and Reablement Team. Cross cover to Urgent Response.
The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with the HCPC.
The pay rate for this locum Social Worker job is £27.27 per hour (LTD Company Equivalent) and is an initial 3-month contract. The hours for this job is Monday to Friday, 9am - 5pm.
Duties:
- Working as part of an MDM in Rehab and Reablement facilitating discharges from the service onto on-going care provision via Care Act assessments/reviews of needs
- Case management for up-to 6 weeks as part of Reablement period of assessment
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current HCPC registration
- Significant senior level frontline experience within adults' services
