What you will do:
– Conduct assessments and provide essential support within the community for adults with mental health needs.
– Collaborate closely with other professionals to ensure comprehensive care and support plans are implemented.
– Respond to MHA assessment referrals, working alongside duty colleagues to deliver timely and effective evaluations.
– Maintain accurate records and documentation to track client progress and service delivery.
– Engage with clients, families, and caregivers to provide education and guidance on available services and interventions.
