What you will do:
- Manage a diverse caseload, undertaking capacity assessments, best interest decision making, Care Act assessments, and participate in a duty rota.
- Deliver outstanding social care to adults and their carers, promoting as much choice and control as possible.
- Engage in strength-based, person-centred assessments with enthusiasm.
- Conduct outcome-focused and preventative conversations, leveraging the strengths of individuals and their local networks.
- Primarily support hospital discharges at either Pinderfields or Dewsbury District Hospitals.
