Qualified Social Worker – Adults Learning Disability Team in Rotherham, earning £33.12 hourly. This is an ongoing, full-time locum position. What you will do:
- Deliver high-quality social care services to vulnerable adults within the local community.
- Conduct thorough investigations and assessments, offering evidence-based reports for complex cases.
- Collaborate with other relevant bodies to perform joint assessments and devise innovative preventative intervention strategies.
- Develop tailored support plans, coordinating the necessary services while maximising available resources.
- Monitor and review ongoing support plans, ensuring they meet industry standards and best practices.
- Educate clients and carers about available local services and support mechanisms.
- Advocate for clients’ dignity, respect, and self-determination while ensuring adherence to safeguarding protocols.
- Maintain accurate records and regularly update clients’ progress through the department’s IT system.
- Partner with local organisations to enhance the multi-agency approach and support services continuity.
