What you will do:
- Assess, develop, and plan specialist treatment for service users with complex mental and physical health issues.
- Provide physiotherapy care to inpatient sites and perform visits to individuals’ homes and care facilities as part of discharge planning.
- Conduct moving and handling assessments for service users presenting with various pathologies and mental health challenges.
- Use advanced investigative and analytical skills to assess service users’ readiness for discharge, identify issues, and develop treatment programs for conditions such as mental illness, stroke, fractures, and Parkinson’s disease.
- Empower service users to maximise their independence and improve their quality of life.
