What you will do:
- Independently assess, treat, and plan the discharge of patients in the community rehab team.
- Utilise recognisable outcome measures and strong clinical reasoning to guide treatment plans.
- Collaborate effectively within a multidisciplinary team to deliver holistic and personalised patient care.
- Drive to various community sites to meet with patients, providing tailored rehabilitation within their home environment.
- Adapt functional activities to enhance clients’ quality of life, making a tangible difference in their day-to-day experiences.
We’re seeking a candidate with at least 12 months of NHS experience and post-qualification as a physiotherapist, preferably within the community and rehab setting. An HCPC registration is necessary, and we highly value car drivers due to the community nature of this role.
