Sanctuary Personnel, a committed and competitive recruitment agency is currently looking for an experienced and dedicated Band 5 Palliative Care Nurse to work based in Barnet.

The salary for this permanent Palliative Care Nurse job is £25,700 - £31,100 per annum.

Shift Pattern:

  • Full Time 37.5 week - to include evening, nights and weekend shifts

Duties:

  • Work in collaboration with other services to provide holistic and clinical care and support to children and young people with life-limiting or life-threatening conditions and their families across a range of settings including community and residential hospice
  • Ensure that holistic and clinical care is child, young person and family centred, needs-led, evidence-based and meets all statutory requirements
  • Assess, plan, implement, evaluate care and end of life care plans to meet the diverse and individual needs of children, young people and families, reporting these back to the wider care team, referral panel and pathway meetings as appropriate
  • Contribute to the management of a caseload of children and young people as directed by the Head of Specialist Care and Nursing
  • Managing referrals, ongoing and annual assessment, care delivery and transition
  • Ensure that all risks associated with care delivery relevant to the care setting are assessed and managed effectively and that all incidents, accidents, safeguarding concerns and complaints are reported promptly following policy and procedure
  • Deliver clinical and holistic care as required
  • Lead service delivery as required
  • Participate in the on-going development of Noah’s Ark Children’s Hospice care and family support services
  • Assess competence of Specialist Carers
  • Participate in an on-call rota within the care team
  • Participate in and lead quality assurance and practice development activities e.g. audit, benchmarking, service review, clinical supervision to ensure that the highest standards of care are maintained
  • Work proficiently and effectively within the relevant legal, ethical and professional frameworks applicable to children and young people’s palliative care practice in the community and in the hospice at all times, including the NMC Code of Professional Conduct
  • Share professional expertise with colleagues, providing support and training where appropriate
  • Be responsible for maintaining and updating own professional practice by actively participating in clinical supervision, appraisal and professional development, participating fully and willingly in opportunities for learning, training and development
  • Act as a mentor, with recognised qualification and updates, to care team members and students, participating in the education and development of Specialist Carers, Nurses, colleagues and students
  • Participate in clinical supervision and provide supervision to volunteers, students, Specialist Carers and wider members of the care team
  • Act as an ambassador for the charity and to maintain positive and effective communication with other services involved with children, young people and families referred to Noah’s Ark Children’s Hospice
  • Undertake any other duties as are within the scope, spirit and purpose of the job, the title of the post and equivalent grading, as requested by your line manager or his/her higher level of authority
  • Work within legal and charity guidelines, ensuring compliance with CQC and other statutory regulations
  • Work within Noah’s Ark Children’s Hospice administrative and IT systems and guidelines and be aware of, and comply with the relevant rules and policies
  • Undertake further duties that may arise as the work of Noah’s Ark continues to develop

Requirements of the Palliative Care Nurse:

  • RN Child / RSCN
  • Active NMC Registration

  • Knowledge of current legislation relating to children & young people including best practice in safeguarding children
  • IT literate (e.g. word processing and E-Mail)
  • Understanding of children’s palliative care and recent initiatives in children’s health and social care
  • Mentorship qualification or willingness to complete qualification
  • Professional knowledge acquired through degree or diploma or other professional training
  • Evidence of working within a professional multi-disciplinary team
  • Experience with children either personally or professionally
  • Evidence of participation in research
  • Experience of providing practice supervision and mentoring
  • Experience of working with volunteers either personally or professionally
  • Evidence of commitment to own continuing professional development
  • Willingness to participate in flexible shift work to include, early mornings, evenings and weekends
  • Willingness to be part of an on-call Rota
  • Have full driver’s licence and own transport
  • Able to assess, develop, implement, deliver and evaluate programmes of care across a range of environments in the community and in the hospice setting
  • Ability to undertake and teach a range of clinical tasks
  • Evidence of a proactive approach to the care delivery process
  • Good written and oral communication skills
  • Ability to reflect on practice and propose changes
  • Ability to problem solve
  • Able to plan and manage own time
  • Ability to work as part of a team
  • Enthusiastic approach to work
  • A flexible attitude and approach to the changing work environment and to provide the flexibility of service required by the families seeking Noah’s Ark services
  • Proven track record of reliability
  • Able and willing to work within a palliative care environment
  • Willing to undertake an enhanced Disclosure and Barring Service Application (DBS)
  • Be aware of the confidential nature of the work of Noah’s Ark Children’s Hospice
