What you will do:
- Assess and evaluate the needs of elderly patients within an acute setting.
- Develop, implement, and review personalised treatment plans.
- Collaborate with multidisciplinary teams to ensure holistic care for your patients.
- Provide education and support to patients and their families as they navigate health challenges.
- Maintain accurate and timely documentation of patient care activities.
To thrive in this role, you need at least 2 years of experience in the acute elderly specialism and be registered with HCPC. A driver’s licence is preferable, enhancing your flexibility and reach across the region.
