Sanctuary Health is currently looking for an experienced and dedicated Band 6 SPA Occupational Therapist/Physiotherapist to work full-time remotely in Aylesbury. The ideal candidate will possess highly developed specialist knowledge of Occupational Therapy/ Physiotherapy and be registered with the HCPC.
The pay rate for this locum Occupational Therapist/Physiotherapist job is dependant on shift (Assignment rate) and is an ongoing contract.
Pay rates:
- Weekday - £24-£25 per hour
- Sat/Nights - £33.10 per hour
- Sun/Bank Holidays - £41.78 per hour
Summary:
- The Single Point of Access (SPA) clinical advisor will work as one of a multi-disciplinary team as part of the Adult Community Healthcare Trust. The overall aims of the integrated teams are to maintain patients in their home environment and avoid inappropriate admission into the acute service as well as facilitate timely discharges from hospital back to their home environment
- The post holder will be responsible for proactively triaging and accepting referrals into the Community services this will involve facilitating discharges from acute services, planning and enabling the pathways of referrals and determining the most appropriate pathway into- community services.
- The post holder will be a registered clinician who will liaise with professional staff within designated clinical areas with the overall aim of preventing delays. The post holder will be proactive in generating solutions to implement appropriate discharges in a safe and timely manner, ensuring the client receives continuity of care through clear and timely communication with all involved in their care.
Requirements of the Occupational Therapist/Physiotherapist:
- Degree or equivalent within Occupational Therapy/ Physiotherapy
- HCPC registered as an Occupational Therapist/ Physiotherapist
- Recent NHS experience either pre or post qualification
