Sanctuary Health is currently looking for an experienced and dedicated Band 6 Discharge to Assess Triage Occupational Therapist/Physiotherapist to work full/part-time based in Wymondham The ideal candidate will possess highly developed specialist knowledge of Occupational Therapy/Physiotherapy and be registered with the HCPC.
The pay rates for this locum Occupational Therapist/Physiotherapist job is £24-£25 per hour (Assignment rate). It is a 4 month contract.
Duties:
- This is an office based role processing and triaging discharge to assess referrals within a multi-disciplinary team
- To work within the Community Response Team to facilitate effective and safe discharges from all inpatient settings.
- To make risk assessments, establishing the safety or otherwise of a patient's ability to stay at home, or return safely from another care setting, providing and arranging supportive packages as necessary.
- To provide evidence based clinical/therapeutic interventions based on 'best practice' in order to improve health outcomes and promote choice.
- To provide in-reach to inpatient units and step up beds where required
- To act as the patients advocate, facilitating choice, patient empowerment and independence.
Requirements of the Occupational Therapist/Physiotherapist:
- Degree or equivalent within Occupational Therapy/Physiotherapy
- HCPC registered as an Occupational Therapist/Physiotherapist
- Recent NHS experience either pre or post qualification
- Clinician preferably with community experience
