What you will do:
- Support patients within the Intermediate Care Rehabilitation Beds, using your specialised skills to aid their recovery
- Develop and implement innovative care plans tailored to each patient’s unique needs
- Collaborate with a multidisciplinary team to ensure holistic patient care and maximise treatment outcomes
- Monitor patient progress and adapt interventions to optimise rehabilitation goals
- Provide education and support to patients and their families, empowering them for a successful transition home
