Sanctuary Health is currently looking for an experienced and dedicated Band 6 Community Occupational Therapist to work in Wales on a Part-time basis. The ideal candidate will possess highly developed specialist knowledge of Occupational Therapy and be registered with the HCPC.
The pay rate for this locum Occupational Therapist job is £27 per hour (Assignment rate) and is an ongoing contract.
This role is part time therefore the candidate will work 3 days a week.
Duties:
- Facilitate and ensure safe early hospital discharge and prevent/reduce any unnecessary long-term permanent placements.
- To manage a defined caseload, using evidence based/client centred principles within physical and psychological medicine.
- To work in collaboration with the multi-disciplinary team and with voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface.
- To maintain clinical records in accordance with legal requirements and professional standards.
- Close liaison with Social Services advising on reducing care needs / ongoing care needs.
- Triage of new referrals.
Requirements of the Occupational Therapist:
- Degree or equivalent within Occupational Therapy
- HCPC registered as an Occupational Therapist
- Recent NHS experience either pre or post qualification
- Community experience
- 2 years' experience
