Sanctuary Health is currently looking for an experienced and dedicated Band 6 Discharge Coordinator/Occupational Therapist specialising in Mental Health to work either full-time or part-time based in Aylesbury. The ideal candidate will possess highly developed specialist knowledge of Occupational Therapy and be registered with the HCPC.
The pay rate for this locum Occupational Therapist job is £25 - £28 per hour (Assignment rate) and is an ongoing contract.
Main Duties/Responsibilities:
- Support with discharging clients that have completed their care and treatment within secondary are mental health services.
- Clients within the adults of working age north service will be transferred to the discharge coordinator to complete necessary actions and outstanding tasks to enable discharge back to primary care and third sector services.
- Strong ability to liaise with other agencies and advocate for clients whilst ensuring their strengths are reflected in discharge plans.
- The post holder will need to be able to work at a fast face to maintain safe patient flow through the service is efficient
Requirements of the Occupational Therapist:
- Degree or equivalent within Occupational Therapy
- HCPC registered as an Occupational Therapist
- Recent NHS experience either pre or post-qualification
- Must be a car driver
