Sanctuary Health is currently looking for an experienced and dedicated Occupational Therapist to work in Brent within a community bedded service on a full-time basis. The ideal candidate will possess highly developed specialist knowledge of Occupational Therapy and be registered with the HCPC.

The pay rate for this locum Occupational Therapist job is £31 - £33 per hour (Assignment rate) and is an ongoing contract.

Duties:

  • Which includes in reach to acute service and outreach to patients in the community planning
  • Be autonomously responsible for assessing and treating own clinical caseload and defining and implementing individualised programmes of care
  • Participate in developing the community rehabilitation service, identifying opportunities for and participating in partnership working within the Trust and all external agencies including GPs, WSIC, Brent CCG
  • Manage junior staff through the day to day line management, supervision and appraisal, to ensure best practice and patient care
  • Lead on clinical audit and research within the specialist area
  • Support the wider operations of ICP, in line with managerial structure and planning
  • Provide the highest standard of evidence-based care to patients within their own home, working with agreed management care plans and protocols
  • Undertake and manage the comprehensive assessment, planning, implementation, evaluation and auditing of clinical care provision in patients own home
  • In collaboration with the therapy lead and manager take responsibility for the day-to-day smooth running of the service and act up as the senior staff member in his/her absence
  • Develop specialist knowledge and skills in the management of long term conditions, working with agreed Trust protocols in collaboration with clinicians and chronic disease practitioners
  • Work in close partnership with hospital colleagues and external agencies such as Social Services, Housing Associations, GPs and voluntary organisations. This will involve attending meetings outside the Trust and sharing protocols and care management plans with the GP's and other agencies
  • Act as a resource for patients/carers, colleagues, GPs and other agencies. This will involve teaching and presentations.

Requirements of the Occupational Therapist:

  • Degree or equivalent within Occupational Therapy
  • HCPC registered as an Occupational Therapist
  • Recent NHS experience either pre or post-qualification
