What you will do:
- Provide high-quality occupational therapy for individuals within the community rehabilitation setting, focusing on early discharge and maintaining independence.
- Conduct assessments and create individualized treatment plans for community rehabilitation and equipment provision.
- Offer guidance and support to clients in reviewing care packages, ensuring they meet the highest standards.
- Work collaboratively with a multidisciplinary team to ensure comprehensive and cohesive patient care.
- Engage in documentation and reporting on patient progress to facilitate successful rehabilitation outcomes.
Requirements:
- Registered with the HCPC.
- A minimum of two years’ experience in community rehabilitation and early discharge.
- Skills in equipment provision and the ability to review care packages effectively.
- Car driving is preferable.
