Sanctuary Personnel, a committed and competitive recruitment agency is currently looking for an experienced and dedicated Band 6 Senior Memory Service Nurse (RMN/CPN) to work full-time based in Barnet.
The pay rate for this locum Senior Memory Service Nurse role is £27 per hour (Assignment rate).
Summary:
- Carry out initial assessments for patients with complex needs referred to Haringey Memory Service.
- Plan, implement and evaluate client care based on robust evidence e.g. NICE guidelines.
- Undertake the individual’s assessment and subsequent care plan as agreed with the multi-disciplinary team.
- Participate in the provision of Post-Diagnostic Interventions for people diagnosed with dementia.
- Provide patients and their carer with a service that is culturally sensitive and gives due consideration to any special needs.
- Support the patient their carer to engage in Advance Care Planning process as part of their care plan.
- Contribute to and lead on continuous improvement initiatives to Haringey Memory Service through audits, quality assurance and patients satisfaction surveys.
- Undertake duties designed to extend clinical and leadership expertise as delegated by the manager when required.
- The post holder will adhere to Trust policies, Data Protection and Health and Safety.
Requirements of the Senior Memory Service Nurse:
- Valid NMC registration under Mental Health
- Previous experience working as a CPN/RMN
- A degree or equivalent within Mental Health Nursing (RMN trained)
