Sanctuary Health has a permanent job vacancy for an experienced and dedicated Hospital Discharge Co-Ordinator to work within a Brain Injury Rehabilitation Centre based in South East London.

The salary for this permanent Hospital Discharge Co-Ordinator role is £22,382.10 - £24,546.60 per annum.

Main Duties/Responsibilities:

  • To ensure patients at Blackheath Brain Injury Rehabilitation Service have well-coordinated discharge care packages alongside effective, seamless discharge plans.
  • To be an active member of the inter-disciplinary team, and provide a comprehensive community liaison service with other health professionals, social services and external agencies.
  • To provide specialist advice and support to patients, carers and relevant external agencies around discharge planning and the discharge process.

Requirements of the Hospital Discharge Co-Ordinator:

  • Hold a recognised professional qualification in Social Work
  • Be registered with HCPC
  • Have detailed knowledge of safeguarding legislation, policies and procedures.
  • Be skilled in developing positive relationships with external and internal departments, family, carers and other stakeholders.
  • Be a confident IT user
  • Able to work as part of a team
  • Able to use regular supervision and training to reflect on practice and develop new skills
  • Be flexible and adaptable, well organised and able to manage time and workload effectively in line with the needs of the service.
  • Have proven knowledge of relevant legislation including Mental Health Act 1983/2007 and the Mental Capacity Act 2005 as it relates to social workers.
