Sanctuary Social Care is currently looking for an experienced and dedicated Early Help Hub Practitioner to work full-time based in Newham. The ideal candidates will possess highly developed specialist knowledge of social work practice and be registered with the HCPC.

The pay rate for this locum Early Help Hub Practitioner role is £20 per hour (LTD Company Equivalent) and is a 3-month contract.

Overall Purpose of the Job:

  • Reviewing allocated requests for support received from partners.
  • Completing research on cases within timescales.
  • Contacting parents / carers to discuss the referral, seek their views and obtain consent for support.
  • Follow up on cases which Early Help Coordinators have allocated for community follow up to ensure refers are aware of the outcome and clear about the work required.
  • Where cases are allocated for Early Help hold a small caseload of families, completing assessment's and developing an early help plan with the family contributing to TAF meetings to hand over plans for delivery in the community.
  • The post holder is part of the Early Help and Families First Service which is committed to working with families to bring about positive and measurable change.
  • The post holder reports to one Early Help Coordinator.
  • The post holder has no line management or budgetary responsibility.
  • The post holder will be required to work on a rota staffing the Early Help Hub from 9 - 5 Monday to Friday.

Duties:

  • To review and research allocated requests for support, contacting families to obtain consent and reaching conclusions on the appropriate plan of early help support required.
  • To be fully conversant with Newham Thresholds for help and protection and to ensure any cases you are researching where safeguarding concerns arise are immediately escalated for review to Early Help Coordinators.
  • To provide targeted support to an allocated caseload of 4-6 families in line with the Early Help principles.
  • To assess family's pre-intervention based on information gathered from parents, children, other family members and other professionals working with the family.
  • To support community partners to develop early help plans for families.
  • To ensure practice is based on a sound evidence-based theory of change with clear outcomes in ng mind.
  • To monitor risk of harm to children and others within and outside the family, seek professional advice and take appropriate action.
  • To make best use of limited financial resources for supporting families working within established procedures.
  • To take part in case conferences, reviews and meetings as appropriate, ensuring that children and young people who use services and their family/carers are supported in making a full contribution to the meetings.
  • To ensure that reports are prepared within allocated timescales and evidence the involvement of children, young people, parents and carers.

Requirements of the Early Help Hub Practitioner:

  • Recent experience within a similar role
  • Graduate in a relevant subject - or experience working with families/children/young people in a holistic help environment
