Sanctuary Health are currently looking for an experienced and dedicated Discharge Care Co-ordinator to work full-time based in Aylesbury. The ideal candidates will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC.
The pay rate for this Care Co-ordinator job is £28-£30 per hour (Assignment Rate) and is an initial 3-month contract.
Job Summary:
- The post holder will support with discharging clients that have completed their care and treatment within secondary are mental health services.
- Clients within the adults of working age north service will be transferred to the discharge coordinator to complete necessary actions and outstanding tasks to enable discharge back to primary care and third sector services.
- Strong ability to liaise with other agencies and advocate for clients whilst ensuring their strengths are reflected in discharge plans.
- The post holder will need to be able to work at a fast face to maintain safe patient flow through the service is efficient
requirements of the Care Co-ordinator:
- Degree or equivalent within Mental Health Nursing (RMN trained)
- NMC registration and a valid live pin
- Recent UK Nursing experience
- Must be a car driver.
- Experience as a practitioner.
- Must be able to manage risk and take positive risks based on the strengths of others.
- Must have experience with complex cases.
