Sanctuary Social Care is currently looking for an experienced and dedicated Service Manager to fill a full-time role within an Adults' Services based in Newham, within the Discharge and Integrated Team.
The pay rate for this locum Service Manager job is £450 per day (LTD Company Equivalent) and is an initial 3-month contract.
Duties:
- To have day to day operational responsibility for ensuring the smooth running of all aspects of discharge planning and processing ensuring local and national standards are achieved. This post will operate across both Health and Social Care and will be accountable for the delivery of the co-ordinated response to demands for improved effective discharge.
- Provide leadership and line management of the hospital based Adult Social Work team and all health funded social work functions within community-based teams such as Virtual Ward, palliative care, neuro rehab and the Casaubon unit, as well as working closely with the site discharge team.
- Support the site lead Operations Director in the delivery of strategic and operational objectives to achieve discharge at the right time, first time.
- To undertake continual review of the hospital adult social work team processes to ensure a co-ordinated and partnership approach to discharge management and minimise delayed transfers of care.
- To lead, manage and support the team through change and the development of new roles and associated ways of working in line with integration.
Requirements:
- A recognised and relevant professional social work qualification (Degree / Diploma in Social Work).
- Registered on Social Work England.
- Management experience is essential.
