Sanctuary Health has a permanent job vacancy for a Band 8a Community Social Care Manager to work full-time with a leading healthcare provider based in Guernsey.

The salary for this permanent Community Social Care Manager role is £48,539 to £56,539 per annum.

Summary:

  • Communicate in a clear and concise manner using tact and persuasive skills where it may be necessary to communicate with service users and other people. Information may be complex and sensitive and the post holder must take action to ensure respect, privacy and dignity of the service user and people in their charge.
  • To manage and provide leadership for integrated multi-professional teams and other community and extra care housing based colleagues, with direct managerial responsibility for senior team members.
  • The primary responsibility will be to enable a high-quality person-centred domiciliary care service which provides direct support to service users with a range of needs in the community using evidence based interventions within agreed care pathways.
  • To ensure service user/carer involvement in the development of standards and services.
  • Manage the team's resources, budget and staff in compliance with the States of Guernsey's policies, procedures & guidelines and statutory responsibilities.
  • Be an integral member of the Community Health & Wellbeing senior leadership team, assisting in the strategic planning, development and delivery of services across the service area.
  • To maintain compliance with, and develop, policies, procedures and guidelines.

Main Duties/Responsibilities:

  • Give respect and consideration to service users respecting individual wishes, aspirations and beliefs. At all times maintain client and department confidentiality as per the HSC policy on Confidential Information.
  • Identifies the range of people involved in communication, potential communication differences, relevant contextual factors and broader situational factors, issues and risks.
  • Communicates with people in a form and manner, which is consistent with their level of understanding, culture, background and preferred ways of communicating.
  • Communicates with people in a form and manner, which is appropriate to the purpose of the communication and its longer-term importance.
  • Communicates with people in a form and manner, which is appropriate to the complexity of the context, encourages effective communication between all involved and enables a constructive outcome to be achieved.
  • Anticipates barriers to communication and takes action to improve communication.
  • Is proactive in seeking out different styles and methods of communicating to assist longer term needs and aims.
  • Takes a proactive role in producing accurate and complete records of communication and communicates in a manner that is consistent with policy and Procedure.
  • To comply with the principles contained in the Bailiwick of Guernsey Data Protection Law (2017) concerning improper disclosure, misuse or breach of confidentiality in respect of information held on computer systems or elsewhere.

Requirements of the Community Social Care Manager:

  • Professional health or social care qualification e.g. Nursing, Social Work, Occupational Therapy, Physiotherapy
  • Registered with the relevant professional body
  • Professional knowledge acquired through degree/diploma supplemented by specialist training or equivalent experience to Master's level equivalent
