Sanctuary Health is currently looking for an experienced and dedicated Band 6 Community Psychiatric Nurse to work full-time as a Primary Care Mental Health Clinician within a Primary Care Mental Health Team based in Uxbridge. The ideal candidate will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC.
The pay rate for this locum Community Psychiatric Nurse role is £25 - £26.50 per hour (Assignment Rate) and is an initial 3-month contract.
Summary:
As post holder of this role you will be working within a service that is responsible for providing effective day to day mental health care assessment and management of clients with mental health problems presenting in Primary Care.
Main Duties:
Receive referrals from GPs and Care Connection Teams in order to undertake comprehensive psycho-socially focused mental health assessments to establish care needs and participate in the screening of referrals within the CCT and support in the identification of those requiring mental health input.
Work as a member of the Care Connection Team (CCT) within the transformed model of case management - Integrated Care Model.
Aim to prevent the escalation of patient's health and/or social care needs thereby reducing the risk of emergency hospital admissions.
The Team provides a single pathway for assessment, referral management and care coordination on behalf of patients with more complex mental health problems presenting in Primary Care.
The post holder will offer short term mental health interventions to support this client group within primary care settings and will work at the primary secondary care interface to facilitate movement of clients between primary and secondary care services.
Provide a dedicated service package to support those newly discharged from secondary care and their GP practice.
A substantial part of their working time will be based within the GP practices of the designated patch.
Requirements of the Community Psychiatric Nurse:
Valid NMC registration under Mental Health
Experience in community mental health desired, preferably 2 years post-qualifying experience
A degree or equivalent within Mental Health Nursing (RMN trained)
