What you will do:
- Manage an allocated caseload and coordinate care plans for mental health service users in community settings.
- Network with relevant teams for smooth transitions of clients between services.
- Safely handle and administer medication, following professional standards and Trust policies.
- Prepare reports and maintain accurate patient records.
- Engage in reflective practice and support junior staff through clinical supervision.
- Lead and participate in meetings to discuss and evaluate client care.
