Sanctuary Personnel, a leading and award winning recruitment agency is currently looking for an experienced and dedicated Band 7 Community Pharmacist to work full-time (37.5 hours per week) based in Surrey.

The pay rate for this locum Community Pharmacist job is £28.99 per hour (Assignment rate) and is a 12-month contract.

Duties:

  • Providing a well-integrated, comprehensive, patient-oriented specialist clinical pharmacy service at ward level throughout SDHC Community Hospital wards
  • Combining pharmaceutical expertise and technical skills to proactively support patient centred medicine optimisation in the Community hospitals
  • Undertaking high quality patient medication reviews to maximise the therapeutic effects of medicines, minimise the risk of harm and improve patient adherence to medication regimens
  • Promoting evidence based, cost effective prescribing by clinicians at the Community Hospitals
  • Maintaining thorough and up-to-date clinical knowledge
  • Supporting the implementation of NICE guidance with a focus on NICE NG5: Medicines Optimisation-the safe and effective use of medicines to enable the best possible outcomes (March 2015)
  • Developing and reviewing policies and Standard Operating Procedures (SOP’s) for use in the Community hospitals for all aspects of Medicine management such as ordering, storage, disposal, administration, prescribing and dispensing
  • Providing support in relation to education and training in relation to medicines management
  • Supervising pharmacy technicians and trainees
  • Participating in Clinical audit where appropriate to aid shared learning and improvement
  • Nurture key relationships, communicate with and provide complex and clinical information to a wide range of internal and external stakeholders
  • Work with appropriate colleagues to develop systems and processes that facilitate seamless admission and discharge in respect of medicines and ensure that medicines optimisation interface issues are addressed
  • Provide specialised medicines optimisation support including evidence based prescribing information, advice and guidance to ensure effective, appropriate, cost-effective and safe prescribing
  • Negotiate changes in prescribing behaviour and influence opinions on prescribing issues
  • Undertake and carry out clinical medication reviews and assessments of patients in the Community Hospitals when required to optimise prescribing and share this information with GP’s and other healthcare professionals to enable patients to obtain optimum outcomes from their medicines
  • Contribute to the wider antimicrobial surveillance programme, ensuring appropriate antimicrobial prescribing in older people, and promote the principle of antimicrobial stewardship and good prescribing practice to colleagues
  • Promoting the safer use of medicines through identification, reporting and investigation of medicines related incidents and sharing the learning from incidents with the wider team
  • Supporting co-owners and patients/ relatives/ carers through distressing or emotional situations arising from medication incidents

Requirements of the Community Pharmacist:

  • Must be registered on the GPHC as a Pharmacist
  • Must have a relevant qualification in pharmacy 
  • Previous pharmacy experience
  • Willing to undertake a DBS check
