Sanctuary Personnel, a committed and competitive recruitment agency is currently looking for an experienced and dedicated Band 6 Community Mental Health Nurse (CPN) to work based in Kingston and Richmond, within the Assessment Team.

The pay rate for this locum role is £28.53 per hour (Assignment rate) and is an initial 1-month contract.

Main Duties:

  • To assess/ triage/ act as first entry point for adults and older adults into community mental health secondary care
  • To assess and work with mental health service users, and signpost them to other services and discharge to primary care where necessary. Any service users needing longer term secondary care and care coordination will be transferred to the relevant teams in the Trust.
  • To work proactively with clients presenting with complex needs, using evidence based /client-centered principles to assess, plan, and direct treatment and discharge plans.
  • The post holder is expected to utilise community resources, working closely with Primary Care.
  • The post holder will ensure user and carer involvement in all aspects of service delivery.  
  • The post holder will be expected to promote, by example and leadership, a high standard of patient care.
  • To regularly supervise nursing students on practice placement.
  • The post holder will communicate and collaborate effectively with all agencies and agents involved in supporting adults with severe mental health problems.

Requirements of the Community Mental Health Nurse:

  • Valid NMC registration under Mental Health.
  • Previous experience working as a CPN.
  • A degree or equivalent within Mental Health Nursing (RMN trained).
  • Mentorship for Contemporary Practice.
  • Demonstrable experience of working with people who experience serious ongoing mental health problems in a variety of settings.
  • Experience of working in a multidisciplinary team.
  • Experience of assessing and managing crisis with those with a severe mental illness.
  • Excellent written and oral communication skills.
  • Skills in assessment, care planning and case management.
  • Ability to assess clinical situations quickly and use initiative.
  • Full and clean driving licence with access to vehicle.
