Sanctuary Personnel, a committed and competitive recruitment agency is currently looking for an experienced and dedicated Band 6 Senior Community Mental Health Nurse (CPN) to work based in Telford, within the Adult Intensive Life Skills Pathway Department.

The pay rate for this locum role is £25 per hour (Assignment rate) and is an initial 1-month contract.

Main Duties:

  • Take responsibility for the management of a complex caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including pharmacological and psychologically informed interventions.
  • Complete and contribute to robust risk assessments and risk management plans.
  • Maintain and effective reporting system by observing and reporting verbally and in writing on patient conditions.
  • Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
  • Assist in ensuring that the aims and objectives of the ward/department (as set by the responsible nurse) are fulfilled and to identify factors which may inhibit these from being achieved.
  • Deliver and promote positive risk taking, where appropriate, to improve quality od life and maximise occupational performance/independence.

Requirements of the Senior Community Mental Health Nurse:

  • Valid NMC registration under Mental Health.
  • Post-registration experience working as a CPN.
  • Evidence of formal courses and training at diploma/degree level.
  • A degree or equivalent within Mental Health Nursing (RMN trained).
  • Evidence of extensive post registration experience with working with adults experiencing mental health related difficulties.
  • Experience of working with people who have difficulty in engaging with services and have complex mental health needs, experience of working in community setting and making decisions autonomously, and experience of leading/contributing to CPA/SAP process.
  • Knowledge of the Mental Health NICE Guidelines, NMC Code of Conduct, and other relevant local and national documents that affects practice.
  • Understanding of the principles of the Care Programme Approach and Community Care Act.
  • Knowledge and experience of holistic assessment of someone’s mental ill health, strengths and needs and appropriate nursing and multi-disciplinary interventions.
