What you will do:
- Plan, implement, review, and improve interventions to meet identified needs and manage inherent risks of the service users.
- Contribute to case conferences and reviews, encouraging service users and their carers to actively engage with services.
- Accept ongoing accountability for service users, manage a delegated caseload of complex cases, and supervise or mentor junior staff in providing exceptional nursing care.
- Provide duty cover, offering support to patients in the CMHT either by telephone or face-to-face interactions.
- Administer intramuscular injections and assess patients who have been referred for mental health support, crafting precise risk management plans and escalating care when necessary.
- Drive to various community settings, including patients’ homes and other sites across the county, delivering top-notch care wherever needed.
