Sanctuary Personnel, a committed and competitive recruitment agency is currently looking for an experienced and dedicated Band 7 Clinical Prison Pharmacist to work full-time based in Wandsworth.
The pay rate for this locum Clinical Prison Pharmacist job is £26 - £27 per hour (Assignment rate) and is a 12-month contract.
Shift Pattern:
37.5 hours (Monday - Friday)
Duties:
Act as Clinical Discharge Pharmacist, rotational with Clinical Pharmacist for Repeat prescribing and MURs
Provide Medicines Management Service for prisoners upon release and transfer and to assist in Medicines Reconciliation at reception
Reconcile medicines at reception and upon discharge from hospital; identify, rectify unexplained changes and manage these changes with and without referring to the GP as appropriate
Produce a post-release medication care plan as required, to be relayed to community GP and Community Pharmacist or healthcare centre of prison if the patient is transferred to it
Liaise with the community Pharmacy to ensure patients receive the medication they need, post discharge
Set up and manage systems to ensure continuity of medication supply to high risk groups of patients e.g. those with medicine compliance aids or other disabilities that may have implications for medicines optimisation
Work in partnership with hospital colleagues to proactively manage patients at high risk of medicine related problems, if they are under the care of such
Identify cohorts of patients at high risk of harm from medicines through pre-emptive, targeted SystmOne search
Review the use of medicines most commonly associated with hospital admissions and re-admissions through audit and individual patient reviews
Assist the clinical Pharmacist for Repeat prescribing and MURs as needed
Devise and implement SystmOne computer searches to identify cohorts of patients at high risk of harm from medicines
Devise and implement SystmOne searches to identify cohorts of patients most likely to be at risk of unplanned admission/readmission to hospital from medicines
Provide leadership to the Head of Healthcare and Clinicians to ensure that practice is compliant with CQC standards where medicines are involved
Contribute to public health campaigns, including flu vaccinations and adult immunisation programmes using specialist knowledge on immunisation
Requirements of the Clinical Prison Pharmacist:
Masters Degree of Pharmacy or equivalent
Registered with the General Pharmaceutical Council of GB (GPhC)
Willing to undertake an advanced DBS check and a prison security check (level 2)
Be familiar with the Trusts infection control policies, and national guidance in relation to infection control
Be aware of the responsibility placed on them by the Health & Safety at Work Act (1974) to ensure that the agreed safety procedures are carried out to maintain a safe condition for employees, patients and visitors
Be familiar with guidance on Child Protection/Safeguarding Children from their own Professional organisations; the trust and the London Safeguarding Children Procedures
Diploma in General Pharmacy Practice/Clinical Pharmacy/Mental Health or equivalent (or working towards)
Evidence of CPD
A member of or working towards Faculty Membership of the Royal Pharmaceutical Society
Registered Independent Pharmacist Prescriber
Previous prison/hospital/community pharmacy experience in the UK
Experience of providing clinical pharmacy services
Experience of providing clinical services within prisons or equivalent
Experience of providing clinical services to mental health or substance misuse patients
Experience of risk management
Experience of working in multidisciplinary groups and providing prescribing advice
Experience of writing guidelines, policies and standard operating procedures
Experience of undertaking patient medication histories and medication reviews
Evidence of activity in research and audit
Recent clinical pharmacy practice in a GP practice or prison setting
Sound clinical skills at diploma level
Demonstrates and appropriate level of clinical reasoning and judgment
Computer literate: Word Processing and Spreadsheet skills
Good communication skills, both written and oral, of complex clinical and drug information
Ability to use own initiative
Prioritise and plan own work effectively and be able to motivate and direct activities of others
Ability to analyse and use clinical evidence and apply it appropriately in different contexts
Good interpersonal and influencing skills, particularly with senior pharmacy and medical staff
Ability to work well under pressure and to meet deadlines
