What you will do:
- Conduct assessments to identify the needs of children with disabilities, developing and implementing care plans accordingly.
- Work closely with families, providing advice and support to enable them to care for their children at home.
- Collaborate with a range of professionals from other sectors to ensure a holistic approach to the child’s care and support.
- Manage a diverse caseload, maintaining accurate and up-to-date records on each case.
- Attend case conferences and reviews, contributing to decision-making processes for each child’s welfare.
