Sanctuary Health has a permanent job vacancy for an Advanced Nurse Practitioner and Prescriber to fill a full-time role in Mansfield. The ideal candidate will possess highly developed specialist knowledge of nursing and be registered with the NMC.
The pay rate for this Advanced Nurse Practitioner role £36.02 per hour.
Job Purpose:
- To proactively respond to patients who have a sudden deterioration in medical functional or social abilities which are likely to result in an inappropriate admission but could be managed in the community.
- To provide urgent care and Rapid Response Service (Days, Nights, weekends and bank holiday days).
- Seek to avoid unnecessary admission to hospital for those patients whose episode of care can be managed within community.
- To work as part of a multidisciplinary/agency Rapid Response team providing a re-enablement service for adults in crisis at home.
- If after a full and comprehensive assessment an admission to hospital is necessary, then the post holder will facilitate the safe and smooth transition of the patient's admission via the appropriate transport.
- Ensure the safe delivery of care for palliative and terminally ill patients within the community.
- To work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the local population.
Requirements of the Advanced Nurse Practitioner:
- Degree or equivalent in nursing
- Current NMC registration
- Must be a prescriber
- Experience within a similar role
