Sanctuary Social Care has permanent vacancy for a Senior Community Practitioner to work full-time based in Guernsey, within the Mental Health Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adult's Services and be registered with Social Work England.

The salary for this permanent Senior Community Practitioner role is £41,956 - £55,315 per annum.

Duties:

  • Provide specialist support and advice to the multidisciplinary team in the management of service users presenting with high risk, dual diagnosis or complex mental health conditions.
  • Ensure that systems and processes are in place to provide integrated care pathways and seamless transitions between departments and services.
  • Will demonstrate knowledge and competence in working with people who have common mental health as well as serious and enduring mental health problems. They will understand the theoretical components of the recovery model and will be able to apply them in practice acting as a role model.
  • Will support the team manager in managing their area of care, including deputizing in their absence.
  • Be responsible for the assessment and care management of service users in a community setting.
  • Manage a full caseload and co-ordinate care for their service users.
  • Carry out comprehensive risk assessment and the formulation of management plans for service users in a community setting.
  • Liaise with other health care professionals including, Psychiatrists, social workers, GP's, Probation, Prison staff, Children's services, In-patient MH services, Emergency Department, Third Sector, Healthy Minds amongst others as required.
  • Participate in the relevant ward reviews and CPA reviews.
  • Participate in the relevant Adult Safeguarding/Child Protection case conferences, and attendance at MAPPA, MARAC and MASH meetings.

Requirements of the Senior Community Practitioner:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant senior level frontline experience within Adult's services
  • Registered Mental Health Nurse
  • Teaching/mentoring qualification
  • Degree in a related field
  • Portfolio evidence to meet agreed competence criteria level Band 6.
  • Evidence of CPD at diploma level.
  • Evidence of teaching/supervisory skills and experience.
  • Evidence of Working with a wide range of Professionals, and ability to work autonomously & Independently
  • Evidence of effective Communication / Interpersonal skills.
  • Evidence of Enthusiasm / Motivation for this role and use of Initiative.
  • Demonstrate Flexibility & Adaptability; be willing to work Out of Hours.
  • Be able to demonstrate good Time Keeping and Time Management skills.
  • Demonstrate the importance of accurate record keeping.
  • Demonstrate a good level of IT skills.
  • Current driving license.
